The Senior Services program provides effective, proactive, and responsive community health services, supporting the physical, mental, and spiritual health of all members. Seniors Services provides comprehensive care for aging community members. Wahta Mohawks aims to promote and maintain independence and living in the home as comfortable , safe, and worry free as possible.
We ensure effective emergency planning, lifeline response, and home safety as well as mobility needs . We provide weekly healthy meals on wheels to senior community members, and we apply DIPPS to our community while providing care. DIPPS (Dignity Independence Privacy Preferences Safety)

Case Management: Provides limited case management (evaluation of social, psychological, and physical needs) and the development of a service plan for a resident when such service is not being provided by the general service community.

Education: Provides educational sessions for health care needs through paramedic services, Community Health Nurses and special services as well as service availability, application procedures, residents rights, etc (both individually and as a group).

Equipment Loaning Program: accessibility equipment is available such as walkers, wheelchairs, crutches, etc. Please contact for the full list of equipment available. Mobility assessments.

Homemakers: Home Cleaning and Organizational Supports and PSW supports.

Seniors Activities: Chair Yoga, Line Dancing, Group Singing, Painting, various trips and craft activities for seniors, social congregate gatherings.

Emergency Response Program: Lifeline 24-hour monitoring service. Also partnership with Bala Taxi, for medical transportation needs.

Interaction: Social interaction/home visits for isolated, frail, vulnerable older adults and seniors

Seniors and Diabetes Program Coordinator

Shé:kon Kawennáhere Sara DeCaire iontiats. I serve as the dedicated Seniors and Diabetes Program Coordinator within the Health and Social Department. In this vital role, I am responsible for overseeing and facilitating key initiatives that support the well-being of seniors and address the prevalence of diabetes within the community.

Program Coordination: Developing, implementing, and managing comprehensive programming specifically tailored to the needs of seniors and individuals living with diabetes. This involves a holistic approach to health, encompassing prevention, management, and support.

Meals on Wheels Facilitation: Playing a crucial role in the facilitation of the “Meals on Wheels” program, ensuring that nutritious meals are delivered to seniors in the community who may have difficulty preparing their own. This service is essential for maintaining their health and independence.

Diabetes-Focused Programming: Leading the development and delivery of specialized programs aimed at diabetes education, prevention, and management. This includes organizing workshops, support groups, and access to resources that empower individuals to better manage their condition and improve their quality of life.

Community Engagement: Collaborating with community members, healthcare providers, and other stakeholders to identify needs, gather feedback, and ensure that programs are culturally sensitive and effective.
My work is instrumental in enhancing the health outcomes and overall quality of life for seniors and those impacted by diabetes in the community.
